Acknowledgment of Risk & Assumption of Liability

I, the undersigned parent/legal guardian of the above-named participant, acknowledge that participation in My Little Ninja involves physical activity and exercise, which may carry an inherent risk of injury. While My Little Ninja instructors take every reasonable precaution to provide a safe and engaging environment, I understand that accidental injuries, such as sprains, strains, bruises, or other minor injuries, may still occur.

By signing this waiver, I voluntarily assume all risks associated with my child's participation and agree that My Little Ninja, its instructors, staff, partnering facilities, and affiliates shall not be held liable for any injury, illness, or other damages that may occur before, during, or after participation in this program.

Medical Treatment Authorization & Media Release

In the event of an emergency where I cannot be reached, I authorize My Little Ninja instructors and staff to administer first aid and/or obtain emergency medical care for my child. I accept full financial responsibility for any medical treatment deemed necessary.

Additionally, I acknowledge that photos and videos may be taken during class for training, promotional, or social media purposes. I grant permission for My Little Ninja to use images or recordings of my child in a respectful manner that highlights the fun and educational aspects of the program. If I wish to opt out, I will provide written notice.

Release of Liability & Hold Harmless Agreement

I agree to release, hold harmless, and waive any claims against My Little Ninja, its owners, coaches, employees, and partnering daycare facilities from any and all liability, claims, demands, or causes of action related to my child’s participation. I further agree that I, my child, and any representatives will not pursue legal action against the program or any associated entities.
